要旨

背景

COVID-19パンデミックから 1年近くが経過したが,デキサメタゾンを除き,死亡率に大きな影響を与える有効な抗SARS-CoV-2薬が未だに不足している.有効な抗ウイルス剤の探索が続く中、我々はコロナウイルスを中心に、ウイルス全般に対する抗寄生虫薬の再利用の可能性に関するデータを検討することを目的とした。

方法

クロロキン,ヒドロキシクロロキン,メフロキン,アルテシミニン,イベルメクチン,ニタゾキサニド,ニコロスアミド,アトバキオン,アルベンダゾールの抗寄生虫薬の抗ウイルス活性を評価した試験管内試験および生体内試験をスクリーニングしてレビューを行った。

結果

ヒドロキシクロロキンおよびクロロキンについては、試験管内試験で抗ウイルス活性の十分な証拠が得られた。COVID-19に対するヒドロキシクロロキンの使用を評価したコホート研究では、相反する結果が報告されているが、ランダム化比較試験(RCT)では、死亡率への影響はなく、COVID-19の予防または治療に ヒドロキシクロロキン(ヒドロキシクロロキン)を使用しても実質的な臨床上の有益性はないことが示された。COVID-19以外のウイルスの治療のためのアルテシミニンの2件の臨床試験とニタゾキサニドの2件の試験を発見したが、いずれも混合した結果を示した。イベルメクチンは1件のRCTと少数の観察研究で評価され、相反する結果が示された。これらのデータのエビデンスレベルが低いため、COVID-19に対するイベルメクチンの有効性はまだ証明されていない。クロロキン、ヒドロキシクロロキン、メフロキン、アルテシミニン、イベルメクチン、ニタゾキサニド、ニコロサミドについては、試験管内試験で様々なウイルスに対する効果が示されている。アトバキオンとアルベンダゾールについては、関連する研究は見つからなかった。

結論

SARS-CoV-2に対して有効な薬剤の探索が続く中で,抗ウイルス活性を有する可能性のある化合物の抗ウイルス効果についての前臨床試験を実施すべきである。臨床研究は試験管内試験で十分な証拠が存在する場合に実施すべきであり、薬物はRCTで厳密に試験された後に広く臨床使用されるようになるべきである。このような探索は、今回のパンデミックにおいても、あるいはこれから発生するアウトブレイクにおいても有益であることが証明されるかもしれない。

序論

COVID-19のパンデミックは、失われた命、世界的な経済不況、政治的緊張、心理的ストレスの面で、すでに衝撃的な代償をもたらした1,2。

既存の抗ウイルス剤の再利用は進行中であるが、これまでのほとんどの研究結果は芳しくなく、多くの薬剤はCOVID-19に罹患する可能性の低い比較的若い患者の少人数グループで有望な結果が得られている。プロテアーゼ阻害剤であるロピナビル/リトナビルは、1つの無作為化比較試験でプラセボに対する明らかな優越性を示すことができなかった。ロピナビル、リトナビル、リバビリン、インターフェロンβ-1bの組み合わせは、症状の緩和とウイルス脱落の短縮に有効であることが確認されているが、このような複雑なレジメンが治療の主力になる可能性は低いと思われる5。

しかし、WHOの単独試験(プラセボ群を含まない)を含む最近の臨床試験では、死亡率の低下は認められなかった7.8。

このような状況下では、試験管内試験での抗ウイルス活性、動物実験、またはCOVID-19患者の予備データに基づいた古い抗寄生虫剤の使用は興味深い選択肢である。本レビューでは、抗マラリア薬を含む抗寄生虫薬のウイルスに対する活性、特にSARS-CoV-2に対する活性に関する利用可能なエビデンスを説明し、COVID-19に対するこれらの治療法の臨床効果を評価した介入研究および観察研究をレビューする。

方法

研究の種類

このレビューでは、COVID-19および他のコロナウイルス関連感染症(MERS、SARSおよびSARS-CoV-1)に対する特異的活性を含む、いくつかの抗寄生虫剤の抗ウイルス活性に焦点を当てた生体内試験および試験管内試験研究をスクリーニングした。抗寄生虫剤には、ヒドロキシクロロキン( ヒドロキシクロロキン)クロロキン、メフロキン、アルテシミニン、イベルメクチン、ニタゾキサニド(NTZ)およびニコロサミド、アトバキオン、およびアルベンダゾールが含まれる。これらの薬剤の構造と用途を表2
に示す。COVID-19 パンデミック期間中に発表された研究を英語で含めた。個々の薬剤の抗ウイルス活性に関するデータを含むRCT、非無作為化比較コホート、症例シリーズ、症例報告を含めた。関連する薬剤と特定のウイルスを用いた実験を詳述している場合には、試験管内試験研究が含まれている。

アウトカム指標の種類

死亡率、入院率、入院期間、重症度などの臨床的に関連するアウトカムが記録された。さらに、ウイルスクリアランス時間、ウイルス排出時間、ウイルス負荷などのウイルス学的パラメータ(関連する場合)も記載されている。

研究の同定のための検索方法

Pubmed検索エンジンを用いて、以下の用語のうち1つ以上を検索して系統的な電子文献スクリーニングを行った。”COVID」、「MERS」、「SARS」、または「抗ウイルス」と、複数の抗寄生虫薬(クロロキン、ヒドロキシクロロキン、メフロキン、アルテミシニン、ニタゾキサニド、ニコロスアミド、イベルメクチン、アルベンダゾール、アトバキオン)の名称を組み合わせて検索した。さらなる研究を特定するために、レビューやシステマティックレビューを含む検索された論文の参考文献を使用した。2人のレビュアーが独立して 2020年11月15日以前に発表されたすべての研究をスクリーニングした。検索された論文のうち、包含基準を満たさないものはすべて除外した。意見の相違があった場合には、3人目の査読者が仲裁人となった。

結果

含められたすべての試験管内試験および生体内試験の論文の数を表1に示す。主な知見を表2と表3にまとめた。アルベンダゾールとアトバキオンについては、試験管内試験も生体内試験もなかったため、これら、2つの薬剤は本レビューには含まれていない。

クロロキンおよびヒドロキシクロロキン

クロロキンと ヒドロキシクロロキンは、4-アミノキノリン系有機化合物で、古くからマラリアの治療や予防、Q熱の抗菌療法、免疫調節剤として使用されていた。これら、2つの薬剤は構造が非常に類似しており、作用機序も同じように見える。 ヒドロキシクロロキンは忍容性が高く、一般的にクロロキンよりも安全性が高いと考えられている。

ヒドロキシクロロキンのほとんどの有害反応は軽度であり、消化管有害作用、皮膚発疹および光線過敏症などがある。一般的に長期にわたって投与される大量の累積投与量は、不可逆的な中毒性網膜症と関連している。9 現在の知見によると、有意な QT 延長は患者の 3~13%で発生し、年齢とともに発生率が上昇し、アジスロマイシンとの併用療法を受けている患者でも発生率が高くなる。COVID-19患者の小規模コホートではトルサード・ド・ポインツは記録されていないが、本剤が広範囲に使用される場合には致死的な不整脈に遭遇する可能性がある10-12。

クロロキンは、ヒト免疫不全ウイルス(HIV)13,C型肝炎ウイルス14,インフルエンザH1N1型15,H5N1型16,デングウイルスやジカなどのフラビウイルス17,中東呼吸器症候群(MERS)ウイルス18-20,およびSARS-CoV-1.21,2223-25など、いくつかのウイルスに対して試験管内試験および動物モデルで活性を示する。

初期の試験管内試験では、クロロキンは低濃度でもSARS-CoV-2感染を阻止するのに有効であることが判明している23 。最近の試験管内試験および生理学的薬物動態学(PBPK)モデリング試験では、SARS-CoV-2を減少させるのにクロロキンよりも ヒドロキシクロロキンの方が高い効力を示した26,27。

ヒドロキシクロロキンとクロロキンの臨床経験

COVID-19 パンデミックの発症から、ほとんどの影響を受けた国で ヒドロキシクロロキン が投与された。投与量と治療期間はガイドラインによって異なるが、400mgを1日2回投与した後、 ヒドロキシクロロキンを200mgを1日2回投与すると、PKモデリングに基づいて抗ウイルス効果が得られると予測された27 。

いくつかの無作為化されていない比較研究または小規模コホートにおいて、クロロキンまたは ヒドロキシクロロキンの使用後に臨床疾患の短縮、ウイルスクリアランスの迅速化、および肺炎発症の可能性の低下が報告されている。29-32 ヒドロキシクロロキンとアジスロマイシンの併用は、非ランダム化小規模非比較研究においても有益であることが示唆されている30,33 。

35-39 ある研究では、 ヒドロキシクロロキン治療を受けた患者では、 ヒドロキシクロロキン治療を受けていない患者に比べて全死因死亡リスクが有意に高かった(調整後ハザード比1.83;95%CI 1.16-2.89;P=0.009)40,40。

最近、3つの大規模RCTの結果が発表された。RECOVERY Collaborative group」試験では、 ヒドロキシクロロキンを投与された患者1561例と標準治療を受けた患者3155例を比較した。28日以内に死亡した患者は、 ヒドロキシクロロキン群で421人(27.0%)であったのに対し、通常ケア群では790人(25.0%)であった(死亡率比、1.09;95%信頼区間[CI],0.97~1.23;P=0.15)。さらに、ベースライン時に機械換気を受けていなかった ヒドロキシクロロキン群の患者では、侵襲的機械換気または死亡の頻度が高かった(30.7%対26.9%;リスク比、1.14;95%CI、1.03~1.27)。死亡は ヒドロキシクロロキン投与患者の10.9%、対照群の9.2%で発生した(リスク比、1.19;95%CI,0.89~1.59;P=0.23)。 ヒドロキシクロロキンの使用は、人工呼吸の開始や入院期間の短縮にはつながらなかった7 。この研究では、プラセボと比較して ヒドロキシクロロキンを投与された患者の14日目の臨床状態スコア(7項目の順序尺度を使用して測定)の分布に有意差はないことが示された(調整後オッズ比、1.02)。

少数の小規模RCTも発表された。そのうちの2件では、 ヒドロキシクロロキン治療群と標準治療群との間にウイルス学的治癒率に有意差がないことが示された43,44,44 。他の3件では、標準治療群と比較して ヒドロキシクロロキン(または ヒドロキシクロロキンとAZ)を使用した場合の死亡率および臨床状態の有益性は観察されなかった45-48。

1件の大規模RCTでは、COVID-19の曝露後予防における ヒドロキシクロロキンの役割が評価された。このRCTでは、COVID-19の新規症例において、 ヒドロキシクロロキン治療(800mgを1回投与し、その後6~8時間で600mgを投与し、さらに4日間毎日600mgを投与する)とプラセボ群との間に有意な差はないことが示された49。

適切なモニタリングなしに ヒドロキシクロロキンを使用しないことを警告する具体的な警告がFDA(アメリカ食品医薬局)から出された50。50 同様に、WHOは最近、 ヒドロキシクロロキンの有益性の欠如を理由にSolidarity Trialを中止した51。

アルテミシニン

アルテミシニンとその誘導体は、植物アルテメシア・アヌアから産生され、マラリアとの戦いにおける主要な武器となっている。アルテシニンの抗マラリア効果は、その完全な作用機序は不明であるが、おそらく活性酸素種の形成に関係していると考えられる。

54,55 アルテシニンとアルテサン酸は、B型肝炎ウイルスとEBVのウイルス産生を抑制した。

アルテシミニンが「偶然の」抗ウイルス剤として広く使用されていることを示す注目すべき実証例が1つある。2014年のリベリアでのエボラパンデミック時には、マラリアとの共感染が疑われる患者には、すべての患者にアルテサン酸-アモジアキンまたはアルテミエーテル-ルメファントリンのいずれかのレジメンが経験的に処方されていた。アルテサン酸-アモジアキンで治療された患者では死亡リスクが31%低かった。この効果はマラリアのない患者でより強く現れていた。

別の研究では、アルテミシア抽出物は細胞病原性効果の減少(これはウイルス自体への効果を証明するものではない)によって測定された SARS CoV-1 に対する活性を示した。最近のin silico試験では、アルテミシニンとSARS-CoV-2スパイクタンパク質Lys353とLys31結合ホットスポットとの間に潜在的な相互作用があることが示されたが、抗ウイルス活性は直接評価されなかった68。これらの試験管内試験およびin silico試験、およびアルテサン酸が動物モデルで敗血症関連肺損傷の減少をもたらしたという事実は、アルテシミニンが抗COVID-19薬としての更なる試験の興味深い候補となる可能性を示唆している。53

メフロキン

メフロキンはキニーネに似た構造の抗マラリア薬である。メフロキンは、Plasmodium寄生虫の無性形態を破壊することで活性を発揮する。過去には、試験管内試験での抗JCウイルス活性により、進行性多巣性脳症の患者に使用されてきた69 。無作為化比較試験では利益が実証されず、抗ウイルス剤としての使用はほとんど中止されている70 。メフロキンは、細胞培養において、SARSのCoV-2に密接に関連する2つのコロナウイルス:パンゴリンコロナウイルスGX_P2V/pangolin/2017/Guangxiおよびネココロナウイルスの細胞病理効果を阻害する71,72。

イベルメクチン

イベルメクチンは、フィラリア症、土壌伝染性蠕虫、疥癬、頭ジラミなど多くの寄生虫感染症の治療に使用されており、比較的安全性が高い。その抗寄生虫効果には、ウイルスとは無関係な寄生虫細胞膜伝染性の上昇が関与している。しかし、デングウイルス、チクングニヤウイルス、およびその他のフラビウイルスに対して試験管内試験での活性を示すことが示されている74-76。タイで実施され、要旨としてのみ発表されたあるRCTでは、デング熱患者においてNS1抗原血症の持続時間が短くなったことが示されたが、臨床的有用性は示されなかった77。

77 試験管内での研究では、イベルメクチンは48時間後にSARS-CoV-2 RNAを5,000倍程度減少させることが示唆されており、これはおそらくウイルスタンパク質のIMPα/β1媒介核輸入を阻害することによるものであろう78。イベルメクチンと ヒドロキシクロロキン の相乗効果も示唆されている。

最近では、ラテンアメリカのほとんどの国では、COVID-19のルーチン治療としてイベルメクチンを使用していたが、まだ実証されていない。79 最近のRCTでは、入院中のCOVID-19患者に対するイベルメクチンまたはイベルメクチンおよびドキシサイクリン療法の効果が評価された。7日後の発熱、咳、咽頭痛はプラセボ群と治療群の間で有意差は認められなかった。イベルメクチン群ではプラセボ群と比較して早期のウイルス学的クリアランスが観察されたが,イベルメクチンおよびドキシサイクリン群では観察されなかった。80

レトロスペクティブコホート研究では、イベルメクチン投与群で標準治療群と比較して死亡率が低く(15.0%対25.2%、OR 0.52,95%CI 0.29-0.96,P=.03)イベルメクチン投与群で重症肺疾患患者の死亡率が低く(38.8%対80.7%、OR 0.15,CI 0.05-0.47,P=.001)機械換気からの離乳成功率に有意差はなかった(36.1%対15.4%、OR 3.11(0.88-11.00,P=.07)。 47, P=.001)が、機械換気からの離乳成功率には有意差はなかった(36.1% vs. 15.4%、OR 3.11 (0.88-11.00), p=.07)。82-84

これらのデータのエビデンスレベルが低い可能性が高いため、COVID-19に対するイベルメクチンの有効性はまだ証明されていない。しかし、現在のエビデンスに基づくと、イベルメクチンがCOVID-19治療のゲームチェンジャーになる可能性は低いと思われる。

ニタゾキサニド

ニタゾキサニド(NTZ)は、ジアルジアやクリプトスポリジウム感染症の治療に用いられる抗寄生虫薬であり、いくつかのウイルスへの再利用の研究が進められている。これらのウイルスには、特に、パラインフルエンザ、呼吸器同期ウイルス(RSV)犬コロナウイルス、ライノウイルス、C型肝炎ウイルス(HCV)を含むフラビウイルス、B型肝炎ウイルス、HIV、インフルエンザなどが含まれている。NTZ は、オセルタミビルやザナミビルとの併用により、インフルエンザ A ウイルスに対して試験管内試験 での相乗効果を示す91 。92 NTZ の活性循環代謝物であるチゾキサニドは、複数のインフルエンザ A および B(インフルエンザ A/H1N1,H3N2,H3N2v、H3N8,H5N9,H7N1 の 16 株、インフルエンザ B の 1 株)の複製を阻害することができる。また,メキシコで入院を要する急性呼吸器疾患患者(6.6%が各種コロナウイルス感染)を対象とした別のRCTでは,プラセボと比較してNTZの効果は認められなかった(93)。

NTZ は、MERS-CoV をはじめとするコロナウイルスに対して、ウイルス性 N タンパク質の発現を阻害することにより抗ウイルス活性を示す94, 95 と、低マイクロモル濃度で SARS-CoV-2 を阻害する。23 COVID-19 の入院患者および外来患者に対する NTZ 治療を評価した小規模なプロスペクティブ非対照試験が 1 件実施された。NTZで治療された20人の女性(妊娠中または妊娠直後の女性)のうち、2人の女性が死亡した。5人の入院患者は良好な転帰を示し、2人の患者は機械的人工呼吸からの離脱が認められた。96 患者数が少なく、死亡率が比較的高いことから、COVID-19に対する本剤の使用を支持するものではない。しかし、より確かなデータを得るためには、より大規模な研究が必要である。

ニクロサミド

ニクロサミド(2′,5-ジクロロ-4′-ニトロサリチルアニリド)は、1958年に発見された抗エストダール活性を持つもう一つのFDA(アメリカ食品医薬局)承認薬である。ニクロサミドは、SARS-CoV-1,97,98 MERS-CoV,99 Zikaウイルス、日本脳炎ウイルス、ヒト免疫不全ウイルス、C型肝炎ウイルス、ヒトアデノウイルスなどの幅広いウイルスに対して試験管内試験での抗ウイルス効果を有している。臨床的な有用性を検討する前に、ニコロサミドの抗ウイルス効果を試験管内試験でさらに評価する必要がある。

議論

現在のCOVID-19パンデミックは、国内および国際的な資源を逼迫させている。研究コミュニティは膨大な量の新しい研究を生み出した。しかし、努力は時に断片化され、実施された多くの臨床試験の方法論は質が低かった。感染発生から数ヶ月が経過した現在、世界中に何百万人もの感染者がいるにもかかわらず、大規模で十分に実施されたRCTの発表数はまだ少ない。有効性が証明されている唯一の抗ウイルス薬であるレムデシビルは、1000人以上の患者を含む大規模RCTで症状の持続時間を短縮することが示されている。この試験では、感染症の基準では絶対的なリスクの低下は小さかったものの、死亡率の低下に向けた有意でない傾向が観察されている。したがって、レムデシビルの効果はわずかであり、より活性の高い抗SARS-CoV-2薬の探索は続いている6。

本レビューでは、試験管内試験または生体内試験で抗ウイルス活性を有することが示されているいくつかの抗寄生虫薬を取り上げた。我々は、新しいSARS-CoV-2とその示唆される作用機序を含む、さまざまなウイルスに対する薬物活性について詳しく説明した(表2,3)。

表2のほとんどの薬剤は、細胞小器官に作用してウイルスの複製段階を減少させることが示された。しかし,これらの薬剤のウイルス,特にSARS-CoV-2に対する作用機序の詳細を明らかにするためには,試験管内試験/生体内試験での集中的な試験が必要である。また,ウイルスのライフサイクルの様々な段階に作用する薬剤の併用療法も検討されるべきである。

このレビューの限界は研究そのものにある。試験管内研究は非常に異なる方法論を用いており、少なくともいくつかの研究は利益相反の可能性のある研究者によって執筆されている。いくつかの臨床研究は、選択バイアス、すなわち比較的予後の良い患者を含めることになりやすい小規模コホートに限定されていた。前述したように、よく実施されたRCTの発表率は残念ながら低く、最近の研究は方法論的または倫理的な問題のために主要な学術誌から撤回されている。

主要なRCTに含まれている抗寄生虫薬は ヒドロキシクロロキンのみである。初期の観察試験では ヒドロキシクロロキンの有用性が示唆されていたが、大規模で質の高い試験では、 ヒドロキシクロロキンはCOVID-19の治療にも予防にも使用すべきではないという十分な証拠が得られている。10 我々は、SARS-COV-2に対する抗ウイルス効果を有する可能性のある既存の化合物の前臨床研究を継続することを求める。しかし、試験管内試験での結果やウイルス学的検査は実際の有効性との相関性が低いことが多いため、薬剤はRCTで厳密に試験された後に初めて広く臨床に導入されるべきであると主張している。このような探索は、今回のパンデミックにおいても、あるいは今後のアウトブレイクにおいても有益であることが証明されるかもしれない。
